RELEASE / WAIVER OF LIABILITY

I waive all rights and claims for damages, injuries & accidents to myself, child & personal property against Dance For HIM for and or any of its representatives of / by myself or child in association with any & all classes / events / camp, etc. which have resulted or may in the future develop from any accident / incident which might occur as a result of before, during or after classes, camp or any association with Dance For HIM and which may arise out of myself / child traveling with, to and from any / all DFH Affiliation. I also authorize Dance For HIM to act on my behalf  according to their best judgment in an emergency requiring medical attention. I give my word of honor that Myself / child are physically able to participate in all activities at / of Dance For HIM Affiliations. I am aware of the risks involved in participating in classes, social, educational and all other events associated with Dance For HIM and hereby assumes all risks & responsibility. This risks include those foreseen & unforeseen, known & unknown.

• Photo and Video Release

I hereby give my permission for Dance For HIM, LLC. to use photos & recorded footage of myself / child during any & all camps, classes and affiliated events / activities. I authorize the use of myself / my child's photos and videos for advertising & website purposes.

          Covid- 19 Guidelines

• Students will not be permitted into nor participate in any affiliated Dance For HIM activity if they have previously tested positive with Covid-19 Virus or symptoms or have been in contact with someone who has Covid-19. 
• Screening will be conducted at the door. Temperatures greater than 100.4 will be sent home.
• Hand sanitizer will be available throughout the facility. 
• No students, visitors, parents or staff will be permitted in / on the Dance For HIM facility. 
• Social Distancing (3 feet apart) will be exercised.
• Mask will be worn except when dancing.
• Space will be provided for belongings.
• Let's be safe and have FUN Dancing!!!
